Subhamol Koottummel
Subha Works as a Social Worker at Toronto Rehabilitation Institute, Canada’s largest leading rehabilitation hospital. She graduated Master of Social Work from India in 2004. Started the career as a Neurology Social worker in an acute care hospital in India.

CASE MANAGER OF THE YEAR

Nicole Galeotalanza
Nicole is the owner and director of Tri-Star Health Management Group Inc. She has been working with families and supporting individuals with traumatic injuries since 2002 and started her case management practice in 2007.

REHABILITATION COMPANY OF THE YEAR

Lawlor Therapy Support Services
Lawlor Therapy Support Services has been providing rehabilitation support services to people with an acquired brain injury and other catastrophic injuries since 1998 in Ontario.

Stacey Levine
Stacey Levine is a behaviour therapist and the co-founder and co-owner of Journey Rehabilitation and Behaviour Therapy; she provides intervention for both children and adults.
